Health and Immunization records are required each summer for all campers including new and returning campers. Submit either the Virginia School Entrance Health Form completed by a doctor, or a school or doctor's form with page one of the Virginia Form completed by a parent and attached to the school/doctor's form. For children age 5 and younger, the doctor's visit must have been within the last 12 months; for all other ages, a copy of the most recent record is acceptable. Deadline for medical forms is June 1.
Complete and return the Medication Authorization Form for non-prescription and prescription medication.
If your child requires prescription medication during the camp day, please complete the Medication Authorization Form. Give the completed form to the camp nurse, along with the medication in its original packaging, labeled with the camper's name and the prescriber's instructions. Medication for camps June 18 - July 13 may be given to the nurse during the week of June 18th; medication for camps July 16 - August 3 may be given to the nurse during the week of July 9th.
If you would like to give the camp nurse to dispense non-prescription medication (such as Tylenol, Benadryl) please complete the Medication Authorization Form and return it to the Summer Programs office.
